Date of Birth

	I am an active Dive Master or Assistant Instructor from an RSTC recognized agency and certified as a PSS Scuba Rescue Diver Plus or equivalent; PSS Deep Diver Plus or equivalent and possess a valid PSS First Aid/CPR and the PSS Oxygen in Diving Emergency or equivalent.
	I have at least six months of diving experience and 80 verifiable logged dives, 10 of which were conducted as a 'dive guide' after having obtained the Divemaster certification
	By the time I complete this course, I will have at least 100 verifiable logged dives. The logged dives will show a broad experience (e.g., advanced, deep, night, navigation, wreck, drift, etc.) since entry level certification, as documented by my logbook/training record or equivalent documentation.
	I will upload in my PSS profile, when established, a medical exam within the last 12 months with approval for diving, without conditions or restrictions, issued by a licensed medical practitioner prior to engaging in water activities.
	I understand that if any of the above prerequisites are not met, I forfeit any payment made toward the completion of this course.
	I am requesting a fee waiver as a qualified US Military Veteran and will provide a copy of my DD214 and/or current Service Identification Card upon request.
